If you lose an important document, follow these tips for obtaining a replacement copy:

Birth Certificate- Go to the vital-statistics office in the birth state. The National Center for Health Statistics website http://tinyurl.com/lszq has phone numbers & addresses of state offices.

Federal Tax Returns- You can order copies from the IRS. Visit http://www.irs.gov to download the required forms or call 800.829.1040 to request them. The cost is $39 for each return requested.

Social Security Card- You can order a replacement at your local Social Security office. Click on the "Contact Us" button at http://www.socialsecurity.gov to find locations. You can also download the application from the website. To prevent identity theft, file your application in person.

Passport- If your passport has been lost or stolen, you must appear in person to request a replacement. The US Department of State's website http://www.travel.state.gov/passport/lost/us/us_848.html has a list of facilities that will accept passport applications (or call 877.487.2778)

Great list Amy. I always worry about what to do with the old tax returns, and how long to keep them. I was happy to see a site where you can order copies.

I think that list came from Real Simple Magazine. I thought it was some really good information as I am always losing something around here Smiley
